Obat ini digunakan bersama obat HIV lainnya dalam terapi kombinasi untuk mengobati infeksi HIV (Human Immunodeficiency Virus). Obat ini juga kadang digunakan sebagai bagian dari strategi pencegahan (PrEP, pre-exposure prophylaxis) bagi individu yang berisiko tinggi tertular HIV.<\/p>\n<p>TENOF-EM tidak menyembuhkan HIV, tetapi bekerja untuk menurunkan jumlah virus HIV dalam darah (viral load), memperkuat sistem imun, dan menurunkan risiko komplikasi infeksi HIV.<\/p>\n<p>&#8212;<\/p>\n<p><strong>Manfaat \/ Indikasi<\/strong><\/p>\n<p>Beberapa kegunaan utama TENOF-EM antara lain:<\/p>\n<ul>\n<li>Sebagai bagian dari pengobatan infeksi HIV-1 bersama obat antiretroviral lain<\/li>\n<li>Digunakan sebagai pencegahan (PrEP) pada orang dengan risiko tinggi tertular HIV, jika dikombinasikan dengan langkah pencegahan lain seperti penggunaan kondom dan pemeriksaan rutin<\/li>\n<\/ul>\n<p>&#8212;<\/p>\n<p><strong>Cara Kerja<\/strong><\/p>\n<p>TENOF-EM mengandung dua jenis obat yang bekerja sebagai reverse transcriptase inhibitors (RTI):<\/p>\n<ul>\n<li><strong>Emtricitabine<\/strong>: merupakan NRTI (nucleoside reverse transcriptase inhibitor)<\/li>\n<li><strong>Tenofovir Disoproxil (TDF)<\/strong>: merupakan bentuk prodrug dari tenofovir, yang setelah dalam tubuh bekerja sebagai <em>nucleotide analog reverse transcriptase inhibitor<\/em><\/li>\n<\/ul>\n<p>Kombinasi ini menghambat enzim reverse transcriptase yang digunakan oleh HIV untuk memperbanyak materi genetiknya. Dengan demikian, replikasi virus dapat ditekan, viral load menurun, dan kerusakan pada sistem imun dapat diminimalkan.<\/p>\n<p>&#8212;<\/p>\n<p><strong>Dosis &amp; Cara Pakai<\/strong><\/p>\n<p><strong>Dosis Umum:<\/strong><\/p>\n<ul>\n<li>Untuk terapi HIV: biasanya <strong>1 tablet 200\/300 mg<\/strong> sekali sehari, dikombinasikan dengan obat antiretroviral lain.<\/li>\n<li>Untuk PrEP: pria\/wanita dewasa dengan berat \u2265 35 kg: 1 tablet per hari, disertai tindakan pencegahan tambahan.<\/li>\n<\/ul>\n<p><strong>Aturan Pakai:<\/strong><\/p>\n<ul>\n<li>Dikonsumsi secara oral, dengan atau tanpa makanan.<\/li>\n<li>Usahakan waktu konsumsi yang sama setiap hari agar tidak terlewat dosis.<\/li>\n<li>Jangan menghentikan obat secara tiba-tiba tanpa persetujuan dokter karena bisa memicu lonjakan virus atau reaktivasi hepatitis B jika ada infeksi HBV laten.<\/li>\n<li>Simpan di suhu ruangan, terlindung dari panas dan kelembapan, jauh dari jangkauan anak-anak.<\/li>\n<\/ul>\n<p>&#8212;<\/p>\n<p><strong>Peringatan &amp; Hal yang Perlu Diperhatikan<\/strong><\/p>\n<p>Sebelum menggunakan TENOF-EM, perhatikan hal-hal berikut:<\/p>\n<ol>\n<li><strong>Alergi Obat<\/strong><br \/>\nJangan gunakan jika Anda memiliki alergi terhadap emtricitabine, tenofovir, atau komponen lain dalam formulasi.<\/li>\n<li><strong>Hepatitis B (HBV)<\/strong><br \/>\nJika Anda memiliki infeksi hepatitis B kronis, penghentian TENOF-EM mendadak dapat memicu reaktivasi HBV yang dapat memperburuk kondisi hati. Dokter akan memantau fungsi hati secara rutin saat menggunakannya dan beberapa bulan setelah penghentian.<\/li>\n<li><strong>Fungsi Ginjal<\/strong><br \/>\nTenofovir dapat memperburuk kondisi ginjal. Sebelum dan selama terapi, dokter akan memeriksa fungsi ginjal (kreatinin, klarifikasi kreatinin) secara berkala.<\/li>\n<li><strong>Asidosis Laktat &amp; Perlemakan Hati<\/strong><br \/>\nPenggunaan obat golongan nukleosida\/nukleotida terkadang dikaitkan dengan asidosis laktat dan hepatomegali dengan steatosis. Bila muncul gejala seperti nyeri otot yang tidak biasa, sesak napas, kelemahan berat, atau nyeri perut, segera konsultasi ke dokter.<\/li>\n<li><strong>Infeksi Tersembunyi \/ Reaksi Imun (Immune Reconstitution Syndrome)<\/strong><br \/>\nSaat sistem imun pulih, bisa timbul reaksi terhadap infeksi laten. Pasien harus diawasi terutama pada fase awal terapi.<\/li>\n<li><strong>Kehamilan &amp; Menyusui<\/strong><br \/>\nPenggunaan selama kehamilan dan menyusui harus disertai pertimbangan manfaat vs risiko. Emtricitabine dapat melalui ASI, sehingga risiko penularan HIV atau efek obat ke bayi harus dievaluasi.<\/li>\n<\/ol>\n<p>&#8212;<\/p>\n<p><strong>Interaksi Obat<\/strong><\/p>\n<p>Beberapa obat atau substansi yang dapat berinteraksi dengan TENOF-EM:<\/p>\n<ul>\n<li>Obat yang memengaruhi fungsi ginjal (misalnya aminoglikosida, penggunaan simultan obat nefrotoksik) \u2192 dapat memperbesar risiko kerusakan ginjal.<\/li>\n<li>Obat yang memicu asam laktat (misalnya interferon alfa) \u2192 potensi akumulasi asam laktat meningkat.<\/li>\n<li>Obat antiretroviral lain, suplemen, atau obat herbal yang dapat mempengaruhi metabolisme atau ekskresi obat \u2013 selalu konsultasikan ke dokter tentang semua obat yang Anda konsumsi.<\/li>\n<\/ul>\n<p>&#8212;<\/p>\n<p><strong>Efek Samping yang Mungkin Terjadi<\/strong><\/p>\n<p>Seperti obat lainnya, TENOF-EM dapat menimbulkan efek samping.
